At first, I was disappointed with some of the reactive responses to adverse events: Nurses on my team were sometimes counseled or written up for being the “origin” of hospital-acquired incidents, even though the changes to our role and cleaning process may have directly related to the uptick in hospital-acquired complications. There had to be a better solution, but the boardroom and the bedside did not seem to understand each other.
